Providers participating in the Accountable Care Coalition of Direct Contracting, an ACO Realizing Equity, Access, and Community Health (REACH) Model (ACO), are on the forefront in the movement to reward coordinated, high-quality care.

As a provider, you will have the opportunity to comprehensively care for your Medicare patients with the support of programs to help improve beneficiary engagement and lower costs, such as greater access to skilled nursing facilities, telehealth services, and beneficiary engagement incentives.

Accountable Care Coalition of Direct Contracting partners with Collaborative Health Systems (CHS), an organization dedicated to enabling providers to operate in a quality-focused, value-based model.

The Accountable Care Coalition of Direct Contracting has been selected by CMS to participate in the ACO Realizing Equity, Access, and Community Health (REACH) Model. Our ACO is committed to meeting the three objectives set forth by the Federal Government: improved care for individuals; improved health for populations; and, lower growth in per capita expenditures.
